As a vital member of a multidisciplinary team, you will deliver comprehensive primary care to patients. You will provide both direct and indirect clinical care under the guidance of medical group administration and care team providers. Exceptional interpersonal skills and critical thinking are essential for prioritizing clinic needs and enhancing patient experiences. You will help build a competent care team focused on quality, patient-centered care, and provider efficiency. Strong time management skills are required, along with a commitment to assist in training staff and attending 80% of scheduled meetings.

Responsibilities

• Prepare for and participate in team meetings, delegating tasks as needed.
  
• Proactively identify clinical needs through monthly quality reports and daily huddles.
  
• Collaborate with team members to set priorities and patient goals, administering depression screenings.
  
• Assess screening needs based on diagnoses.
  
• Update health maintenance records and inform providers of patient needs.
  
• Manage referrals and answer calls in real-time when possible.
  
• Complete Annual Wellness Visits under provider supervision.
  
• Engage in patient education, including goal setting and self-management.
  
• Assist with staffing coordination and maintain clinic supplies and cleanliness.
  
• Evaluate clinic for quality compliance and submit outdated checklists monthly.
  
• Act as a preceptor for new staff, orienting them to the clinic.
  
• Attend Clinic Coordinator meetings and follow organizational operations.
  

Qualifications

KNOWLEDGE/EXPERIENCE: Preferred 2 years of clinic experience and oversight of unit operations.

EDUCATION: Current Montana License as a Licensed Practical Nurse.

LICENSE/CERTIFICATION/REGISTRY: Current Montana State Board of Nursing licensure as LPN; Basic Life Support (CPR) certification required, with re-certification every two years.
