PRINCIPAL DUTIES:
- Assess patients’ psychosocial status and needs.
- Collaborate with the healthcare team to set treatment goals.
- Utilize educational programs and interventions to address barriers.
- Provide counseling and community resource referrals.
- Facilitate discussions on advance directives and DNR status.
- Educate patients and families on ESRD-related issues.
- Train clinic staff on psychosocial topics.
EDUCATION:
- Master’s in Social Work
- State-required license
EXPERIENCE:
- 0-2 years related experience
PHYSICAL DEMANDS:
- Travel may be required.
Salary: $56,000 – $88,000 based on qualifications. EO/AA Employer.
