Job Description: Post-Acute Long-Term Care Nurse Navigator
The Staff Pad is collaborating with a client in Montana aiming to set the gold standard in healthcare. We are seeking a Post-Acute Long-Term Care Nurse Navigator to further the organization’s mission. This dynamic leader will be essential in coordinating patient care within skilled nursing facilities (SNFs), ensuring top-quality service for patients and their families.
Responsibilities:
Advocate for patients, enhancing their healthcare journey and outcomes.
Regularly visit local SNFs to ensure continuity of care and foster relationships with care teams.
Work with multidisciplinary teams to develop and implement personalized care plans.
Coordinate outpatient and virtual visits, ensuring timely scheduling.
Maintain accurate EMR documentation and facilitate communication between SNF staff and providers.
Educate SNF staff on operations and care topics.
Identify and implement strategies for improving care processes and patient experiences.
Qualifications:
3-5 years of nursing experience, with at least one year in acute care preferred.
Graduate of an accredited nursing program; BSN preferred.
Current Montana RN license and BLS certification required.
Strong communication, time management, and collaboration skills.
