We are seeking a compassionate and dedicated Licensed Vocational Nurse (LVN) for our Home Health team in Carlsbad, CA.
**Responsibilities:**
Provide direct patient care in home settings.
Administer medications and treatments as prescribed.
Monitor patient health and report changes to the healthcare team.
Educate patients and families on care plans and health management.
Maintain accurate patient records and documentation.
**Qualifications:**
Current LVN license in California.
Experience in home health preferred.
