We are seeking a dedicated Physical Therapist to join our home health team in Lancaster, CA. The ideal candidate will provide high-quality, patient-centered care in a home setting.
**Responsibilities:**
Conduct comprehensive assessments and develop individualized treatment plans.
Implement therapeutic interventions to improve mobility and function.
Educate patients and families on exercises and health management.
Document patient progress and adjust treatment plans as necessary.
Collaborate with interdisciplinary teams to ensure optimal patient outcomes.
**Qualifications:**
Licensed Physical Therapist in California.
Experience in home health preferred.
Strong communication and interpersonal skills.
