**Salary:** $38.76 – $71.40 per hour
**Job Description:**
We are seeking a dedicated RN for our Per Visit CHHA program. The ideal candidate will provide high-quality nursing care to patients in their homes.
**Responsibilities:**
Conduct patient assessments and develop care plans.
Administer medications and treatments as prescribed.
Educate patients and families on health management.
Collaborate with interdisciplinary teams to ensure optimal care.
Document patient progress and maintain accurate records.
**Qualifications:**
Current NY RN license.
Experience in home health preferred.
Strong communication and organizational skills.
