**Job Title:** Part-Time Health Home Behavioral Health Consultant **Location:** Springfield, MO **Salary:** $25 – $30 per hour **Job Description:** We are seeking a dedicated Part-Time Health Home Behavioral Health Consultant to join our team in Springfield, MO. The ideal candidate will provide comprehensive behavioral health support to clients within a health home model. **Responsibilities:**

• Conduct assessments and develop individualized care plans.

• Collaborate with healthcare providers to ensure holistic care.

• Facilitate group and individual therapy sessions.

• Monitor client progress and adjust treatment plans as needed.

• Provide education and resources to clients and families.

**Qualifications:**
  • Master’s degree in Social Work, Psychology, or related field.
  • Valid state licensure (LCSW, LPC, etc.).
  • Experience in behavioral health settings preferred.
