**Job Title:** RN Case Manager **Location:** Durham, NC **Salary:** $74,880.00 – $81,883.72 per year

We are seeking a dedicated RN Case Manager to join our team in Durham County. The ideal candidate will be responsible for:

  • Assessing patient needs and developing individualized care plans.
  • Coordinating services across healthcare providers to ensure optimal patient outcomes.
  • Monitoring patient progress and adjusting care plans as necessary.
