We are seeking a compassionate LPN to join our hospice team in Cambridge, OH. The ideal candidate will provide high-quality nursing care to patients in their homes or hospice facilities.
**Responsibilities:**
Administer medications and treatments as prescribed.
Monitor patient vital signs and report changes to the healthcare team.
Assist with daily living activities and provide emotional support to patients and families.
Document patient care and maintain accurate records.
Collaborate with interdisciplinary teams to develop care plans.
**Qualifications:**
Current LPN license in Ohio.
Experience in hospice or palliative care preferred.
Strong communication and interpersonal skills.
