**Job Title:** Registered Nurse (RN) Hospice Case Manager

**Salary:** From $48/hour
**Job Description:**
We are seeking a compassionate and dedicated RN Hospice Case Manager to join our team in Salem, OR.
**Responsibilities:**
Provide comprehensive nursing assessments and develop individualized care plans for hospice patients.
Coordinate with interdisciplinary teams to ensure quality end-of-life care.
Educate patients and families on hospice services and symptom management.
Monitor patient progress and adjust care plans as needed.
Document all patient interactions and care provided in compliance with regulations.
**Qualifications:**
Current RN license in Oregon.
Previous hospice or palliative care experience preferred.
Strong communication and organizational skills.
