**Job Title:** Physical Therapist (PT) – Home Health **Location:** San Francisco, CA **Salary:** $108,000 – $189,000 per year

We are seeking a dedicated Physical Therapist (PT) for our Home Health team in San Francisco. The ideal candidate will provide high-quality, patient-centered care in a home setting.

Responsibilities:

• Conduct comprehensive assessments and develop individualized treatment plans.
• Implement therapeutic interventions to improve mobility and function.
• Educate patients and families on rehabilitation techniques and home exercises.
• Document patient progress and adjust treatment plans as necessary.

Qualifications include a valid PT license in California and experience in home health preferred.
