**Salary:** $75,000 – $85,000 per year
**Job Description:**
We are seeking a compassionate Hospice Registered Nurse Case Manager to join our team in Sioux City, IA. The ideal candidate will provide holistic care to patients and their families during end-of-life transitions.
**Responsibilities:**
Conduct comprehensive patient assessments and develop individualized care plans.
Coordinate with interdisciplinary teams to ensure quality care delivery.
Educate patients and families on hospice services and support options.
Monitor patient progress and adjust care plans as needed.
Document all patient interactions and maintain accurate records.
**Qualifications:**
Current RN license in Iowa.
Previous hospice or palliative care experience preferred.
Strong communication and organizational skills.
