**Job Title:** Home Health Physical Therapist (PT) – Full-Time **Location:** Sacramento, CA **Salary:** $46 – $77 per hour

We are seeking a dedicated Home Health Physical Therapist to join our team in Sacramento. The ideal candidate will provide high-quality, patient-centered care in home settings.

  • Responsibilities:
    • Conduct comprehensive assessments of patients’ physical abilities.
    • Develop and implement individualized treatment plans.
    • Educate patients and families on rehabilitation exercises and techniques.
    • Monitor progress and adjust treatment plans as necessary.
    • Collaborate with healthcare teams to ensure optimal patient outcomes.

    Qualifications include a valid PT license in California and experience in home health settings preferred.
