Job Description

**Job Title: Hospice Registered Nurse Case Manager** **Location: Ontario, CA** **Salary: $45 – $52 per hour**

We are seeking a compassionate and dedicated Hospice Registered Nurse Case Manager to join our team. The ideal candidate will provide holistic care to patients and their families during end-of-life transitions.

  • Responsibilities:
    • Conduct comprehensive patient assessments and develop individualized care plans.
    • Coordinate with interdisciplinary teams to ensure quality care delivery.
    • Educate patients and families on hospice services and symptom management.
    • Document patient progress and adjust care plans as needed.
    • Provide emotional support and counseling to patients and families.

    Qualifications: Current RN license in CA, hospice experience preferred.
