We are seeking a compassionate RN/LPN to provide high-quality home care to patients in Monroe, NY.
**Responsibilities:**
Administer medications and treatments as prescribed.
Monitor patient health and report changes to the healthcare team.
Assist with daily living activities and personal care.
Educate patients and families on health management.
Maintain accurate patient records and documentation.
**Qualifications:**
Current RN/LPN license in NY.
Previous home care experience preferred.
