Sippican Healthcare Center is seeking a dedicated MDS Nurse (RN) to join our team. The ideal candidate will be responsible for:
Conducting comprehensive assessments and completing MDS documentation.
Collaborating with interdisciplinary teams to develop and implement care plans.
Ensuring compliance with state and federal regulations.
Monitoring patient progress and adjusting care plans as needed.
Providing education and support to patients and families.
