Job Description

**Job Title:** Hospice Registered Nurse Case Manager **Location:** Fond du Lac, WI **Salary:** $75,000 – $85,000 per year

We are seeking a compassionate and dedicated Hospice Registered Nurse Case Manager to join our team in Fond du Lac, WI. The ideal candidate will provide holistic care to patients and their families during end-of-life transitions.

  • Responsibilities:
    • Conduct comprehensive patient assessments and develop individualized care plans.
    • Coordinate with interdisciplinary teams to ensure quality care delivery.
    • Educate patients and families on hospice services and support options.
    • Monitor patient progress and adjust care plans as needed.

    Qualifications include an active RN license, hospice experience preferred, and strong communication skills.
