We are seeking a compassionate Hospice LPN to join our dedicated team in Litchfield, MN. The ideal candidate will provide high-quality nursing care to patients in hospice settings.
**Responsibilities:**
Administer medications and treatments as prescribed.
Monitor patient vital signs and report changes to the healthcare team.
Assist with daily living activities and provide emotional support to patients and families.
Document patient care and maintain accurate records.
Collaborate with interdisciplinary teams to develop care plans.
**Qualifications:**
Current LPN license in Minnesota.
Experience in hospice or palliative care preferred.
