**Job Description:**
We are seeking a Bilingual Risk Adjustment Nurse Practitioner to join our healthcare team. The ideal candidate will be responsible for:
Conducting comprehensive assessments of patients to identify risk factors.
Collaborating with interdisciplinary teams to develop care plans.
Providing education to patients and families in both English and Spanish.
Reviewing medical records for accuracy and compliance with risk adjustment protocols.
Participating in quality improvement initiatives to enhance patient outcomes.
Qualifications include an active NP license, bilingual proficiency, and experience in risk adjustment or managed care.
