**Job Title:** RN Case Manager – Home Health & Hospice
**Location:** Duchesne, UT
**Salary:** $36 – $39 per hour
**Job Description:**
We are seeking a compassionate RN Case Manager for our Home Health & Hospice team. The ideal candidate will provide comprehensive patient care, ensuring quality and continuity of services.
**Responsibilities:**
Conduct patient assessments and develop individualized care plans.
Coordinate with interdisciplinary teams to deliver holistic care.
Educate patients and families on health management and hospice services.
Monitor patient progress and adjust care plans as needed.
Document all patient interactions and maintain accurate records.
**Qualifications:**
Current RN license in Utah.
Previous experience in home health or hospice preferred.
Strong communication and organizational skills.
