**Job Title:** RN/Case Manager for Home Health Care (Part-Time, Pay Per Visit) **Location:** Colorado Springs, CO **Salary:** From $35/hour

We are seeking a dedicated RN/Case Manager for our Home Health Care team. This part-time position offers flexible scheduling and pay per visit compensation.

  • Responsibilities:
    • Conduct patient assessments and develop care plans.
    • Coordinate with healthcare providers and families.
    • Monitor patient progress and adjust care plans as needed.
    • Provide education and support to patients and caregivers.

    Qualifications:

    • Current RN license in Colorado.
    • Previous home health experience preferred.
