We are seeking a dedicated RN Case Manager for our Home Care team in Greenwood, SC. The ideal candidate will provide comprehensive nursing care and case management for patients in their homes.
**Responsibilities:**
Conduct patient assessments and develop individualized care plans.
Coordinate with healthcare providers and family members to ensure optimal patient outcomes.
Monitor patient progress and adjust care plans as necessary.
Educate patients and families on health management and resources.
**Qualifications:**
Current RN license in South Carolina.
Previous home care or case management experience preferred.
Strong communication and organizational skills.
