**Job Title:** RN MDS Assessor
**Location:** Brooklyn, NY
**Salary:** $90,000 – $95,000 per year
**Job Description:**
We are seeking a dedicated RN MDS Assessor to join our team in Brooklyn. The ideal candidate will be responsible for:
Conducting comprehensive assessments of residents using the Minimum Data Set (MDS) to ensure compliance with federal and state regulations.
Collaborating with interdisciplinary teams to develop and implement individualized care plans.
Monitoring and evaluating resident progress and adjusting care plans as necessary.
Providing education and support to staff and families regarding MDS processes and resident care.
**Qualifications:**
– Current RN license in NY
– Strong knowledge of MDS and regulatory requirements
– Excellent communication and organizational skills
