We are seeking a dedicated RN Case Manager to join our team in Rapidan. The ideal candidate will be responsible for:
Assessing patient needs and developing individualized care plans.
Coordinating services across healthcare providers to ensure optimal patient outcomes.
Monitoring patient progress and adjusting care plans as necessary.
Educating patients and families on health management and resources.
Documenting all interactions and maintaining accurate patient records.
**Qualifications:**
– Current RN license in Virginia
– Strong communication and organizational skills
– Experience in case management preferred
