**Job Title:** Hospice RN Case Manager – Full Time Days **Location:** Basking Ridge, NJ **Job Description:** We are seeking a compassionate and dedicated Hospice RN Case Manager to join our team. This full-time position focuses on providing high-quality care to patients in a home-based setting. **Responsibilities:**

• Conduct comprehensive patient assessments and develop individualized care plans.

• Coordinate with interdisciplinary teams to ensure holistic patient care.

• Educate patients and families on hospice services and end-of-life care.

• Monitor patient progress and adjust care plans as needed.

• Maintain accurate documentation and compliance with regulations.

**Qualifications:**
  • Current RN license in NJ
  • Experience in hospice or palliative care preferred
  • Strong communication and organizational skills
