**Job Title: Home Health Physical Therapist – Full Time** **Location: Rocky River, OH** **Salary: $85,000 – $92,000 per year** We are seeking a dedicated Home Health Physical Therapist to join our team in Rocky River, OH. This role involves providing high-quality physical therapy services to patients in their homes, promoting recovery and enhancing quality of life.

Responsibilities:

Conduct patient assessments and develop individualized treatment plans.

Implement therapeutic interventions to improve mobility and function.

Educate patients and families on home exercise programs and safety.

Document patient progress and adjust treatment plans as needed.

Requirements:

Valid Ohio Physical Therapy License.

Experience in home health preferred.
