This role involves providing specialized care to patients with neurological conditions in their homes, ensuring they receive the highest quality of therapy.

Responsibilities:

Conduct comprehensive assessments of patients’ physical abilities.

Develop and implement individualized treatment plans.

Monitor patient progress and adjust therapies as needed.

Educate patients and families on home exercises and safety.

Requirements:

Valid California PT license.

Experience in neuro rehabilitation preferred.

Strong communication and interpersonal skills.

Ability to work independently in a home setting.
