Job Title: Licensed Physical Therapist – SNF (PT) Part-Time or Full-Time Location: San Leandro, CA Salary: $44 – $65 per hour

We are seeking a dedicated Licensed Physical Therapist to join our skilled nursing facility (SNF) team in San Leandro, CA. This position offers both part-time and full-time opportunities.

Responsibilities:

Conduct patient assessments and develop individualized treatment plans.

Implement therapeutic interventions to improve patient mobility and function.

Collaborate with healthcare team members to ensure optimal patient care.

Document patient progress and adjust treatment plans as needed.

Qualifications:

Valid California PT license.

Experience in a SNF setting preferred.
