Job Description

**Job Title:** Home Health Physical Therapist **Location:** City, TX **Salary:** $55 – $60 per hour

We are seeking a dedicated Home Health Physical Therapist to join our team. The ideal candidate will provide high-quality, patient-centered care in a home setting. Responsibilities include:

  • Conducting comprehensive assessments of patients’ physical abilities
  • Developing and implementing individualized treatment plans
  • Educating patients and families on exercises and rehabilitation techniques
  • Collaborating with healthcare professionals to optimize patient outcomes
  • Documenting patient progress and adjusting treatment plans as necessary

    • Qualifications: Licensed PT in TX, strong communication skills, and experience in home health preferred.
