**Job Title:** PRN Home Health Physical Therapist

**Salary:** $100 – $140 per hour
**Job Description:**
We are seeking a dedicated PRN Home Health Physical Therapist to provide high-quality care to patients in their homes.
**Responsibilities:**
Conduct comprehensive evaluations and develop individualized treatment plans.
Implement therapeutic interventions to improve mobility and function.
Educate patients and families on home exercise programs and safety.
Document patient progress and adjust treatment plans as needed.
Collaborate with healthcare teams to ensure optimal patient outcomes.
**Qualifications:**
Valid California PT license.
Experience in home health preferred.
Strong communication and interpersonal skills.
