**Position Overview:** This role involves conducting biopsychosocial assessments, providing crisis intervention, offering short-term counseling, linking patients with resources, and planning for transitions of care.

**Responsibilities:**

  • Perform comprehensive assessments of patients’ needs.
  • Provide crisis intervention and support.
  • Facilitate access to community resources.
  • Develop care transition plans.

  • Master’s degree in social work or related field.
  • Valid licensure in California.
  • Strong communication and interpersonal skills.
