**Description** **The Position:** The Intake Specialist I (IS I) will facilitate the intake and enrollment process, guiding participants to their designated center’s Interdisciplinary Team (IDT) for capitation. Collaborating with the Enrollment Development Representative (EDR), the IS I will help enhance PACE program membership. **Responsibilities:**

• Conduct initial assessments and gather necessary documentation.

• Provide information about the PACE program to potential participants.

• Coordinate with IDT members to ensure smooth transitions.

• Maintain accurate records and follow up on enrollment status.

**Requirements:**
  • High school diploma or equivalent.
  • Strong communication and interpersonal skills.
  • Experience in healthcare or social services preferred.
  • Proficient in Microsoft Office Suite.
