Job Description

Licensed Practical Nurse (LPN) – Home Care
Experience independence and flexibility while providing one-on-one care to patients in their homes.

Hours: Full-time | Days 8:00am – 5:00pm | .8 FTE (32 hours) Fri/Sat/Sun/Mon OR 1.0 FTE (40 hours) Thurs/Fri/Sat/Sun/Mon

Must-Haves:

  • Current LPN license in MN.
  • 2 years of LPN experience.
  • Valid driver’s license, reliable transportation, and proof of auto insurance.
  • CPR Certification.

Key Responsibilities:

  • Participate in ongoing patient assessments.
  • Develop and evaluate care plans with patients and caregivers.
  • Provide physician-ordered services.
  • Report changes in patient conditions to RN supervisor.
  • Educate patients and families on healthcare needs.
  • Document clinical notes accurately and timely.
