Job Description

PURPOSE AND SCOPE:
This role provides psychosocial services to patients, including those undergoing in-center and home dialysis, using Social Work Theory and practice. Collaborating with the healthcare team, the goal is to enhance patient adjustment, rehabilitation, and quality of life. The position supports Fresenius Kidney Care’s commitment to quality outcomes and patient satisfaction, participating in improvement activities. This is an entry-level MSW position.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

  • Conduct patient assessments and care planning, addressing psychosocial needs.
  • Provide counseling and education tailored to patients and families.
  • Facilitate referrals to community resources and vocational rehabilitation.
  • Participate in quality review meetings and document interventions.
  • Assist with insurance and financial assistance issues.
  • Collaborate with staff on psychosocial training and patient care.

    • EDUCATION AND REQUIRED CREDENTIALS:

  • Master’s in Social Work.
  • State-required license.

    • EXPERIENCE AND SKILLS:

  • 0-2 years of related experience.
