We are seeking a compassionate Home Care Occupational Therapist to join our team in Rogers, MN. The ideal candidate will provide personalized therapy services to clients in their homes, enhancing their ability to perform daily activities.
**Responsibilities:**
Conduct assessments to develop individualized treatment plans.
Implement therapeutic interventions to improve clients’ functional abilities.
Educate clients and families on adaptive techniques and equipment.
Document progress and adjust treatment plans as needed.
**Qualifications:**
Master’s degree in Occupational Therapy.
Current MN OT license.
Strong communication and interpersonal skills.
