We are seeking a compassionate and dedicated LPN to join our team at Moweaqua Rehabilitation & Health Care Center.
**Responsibilities:**
Provide direct patient care under the supervision of registered nurses and physicians.
Administer medications and treatments as prescribed.
Monitor patient health and report changes to the healthcare team.
Assist with daily living activities and ensure patient comfort.
Maintain accurate patient records and documentation.
**Qualifications:**
Current LPN license in Illinois.
Strong communication and interpersonal skills.
Ability to work in a team-oriented environment.
