**Job Title:** On-Call Hospice Registered Nurse Case Manager **Location:** Fond du Lac, WI **Salary:** $75,000 – $85,000 per year **Job Description:** We are seeking a compassionate On-Call Hospice Registered Nurse Case Manager to join our team. This role involves providing high-quality end-of-life care to patients and support to their families. **Responsibilities:**

• Conduct patient assessments and develop individualized care plans.

• Administer medications and treatments as prescribed.

• Provide emotional support and education to patients and families.

• Collaborate with interdisciplinary teams to ensure comprehensive care.

• Respond to on-call needs promptly and effectively.

**Qualifications:**
  • Current RN license in Wisconsin
  • Experience in hospice or palliative care preferred
  • Strong communication and organizational skills
