**Salary:** $70,000 – $85,000 annually

We are seeking a dedicated RN Case Manager to join our healthcare team in Fremont, NE. The ideal candidate will be responsible for:
Assessing patient needs and developing individualized care plans.
Coordinating care among healthcare providers and community resources.
Monitoring patient progress and adjusting care plans as necessary.
Educating patients and families on health management and resources.
Documenting patient interactions and outcomes in compliance with regulations.
**Qualifications:**
– Current RN license in Nebraska
– Strong communication and organizational skills
– Experience in case management preferred
