**Job Title:** Hospice RN Case Manager
**Location:** Natick, MA
**Salary:** $105,000 – $120,000 per year
**Job Description:**
We are seeking a compassionate Hospice RN Case Manager to join our dedicated team in Natick, MA. The ideal candidate will provide holistic care to patients and families facing end-of-life challenges.
**Responsibilities:**
Conduct comprehensive patient assessments and develop individualized care plans.
Coordinate with interdisciplinary teams to ensure quality care delivery.
Educate patients and families on hospice services and support options.
Monitor patient progress and adjust care plans as needed.
Document all patient interactions and maintain accurate records.
**Qualifications:**
Current RN license in Massachusetts.
Minimum 2 years of nursing experience, preferably in hospice or palliative care.
Strong communication and organizational skills.
