**Schedule:** 7on/7off, 7a to 7p
**Job Description:**
We are seeking a dedicated Acute Care APRN to join our Hospital at Home program. This role involves providing high-quality, patient-centered care to individuals in their homes.
**Responsibilities:**
Conduct comprehensive assessments and develop care plans.
Manage acute and chronic conditions in a home setting.
Collaborate with interdisciplinary teams to ensure optimal patient outcomes.
Educate patients and families on health management.
**Qualifications:**
Master’s degree in Nursing.
ACNP or AGACNP certification required.
Active Oklahoma APRN license.
