**Job Title: Home Care RN Case Manager** **Location: Zimmerman, MN** **Salary: $70,000 – $110,000 per year**

We are seeking a dedicated Home Care RN Case Manager to join our team in Zimmerman, MN. The ideal candidate will be responsible for:

Conducting comprehensive patient assessments and developing individualized care plans.

Coordinating and managing patient care services in collaboration with healthcare providers.

Monitoring patient progress and adjusting care plans as necessary.

Providing education and support to patients and their families.
