We are seeking a dedicated Registered Nurse to join our Oncology Care Coordination team. The ideal candidate will provide comprehensive patient care, ensuring seamless communication between patients, families, and healthcare providers.
**Responsibilities:**
Assess and coordinate patient care plans.
Educate patients on treatment options and side effects.
Collaborate with multidisciplinary teams to optimize patient outcomes.
Monitor patient progress and adjust care plans as needed.
**Qualifications:**
Current RN license in Minnesota.
Experience in oncology preferred.
