**Job Title:** RN Case Manager – Hospice **Location:** Pueblo, CO **Salary:** $35 – $38 per hour **Job Description:** We are seeking a compassionate RN Case Manager for our hospice team in Pueblo, CO. The ideal candidate will provide holistic care to patients and families facing end-of-life challenges. **Responsibilities:**

• Conduct comprehensive assessments and develop individualized care plans.

• Coordinate with interdisciplinary teams to ensure quality care.

• Educate patients and families on hospice services and resources.

• Monitor patient progress and adjust care plans as needed.

• Maintain accurate documentation and compliance with regulations.

**Qualifications:**
  • Current RN license in Colorado
  • Previous hospice or palliative care experience preferred
  • Strong communication and organizational skills
