We are seeking a dedicated Licensed Physical Therapist to join our team in Lake in the Hills, IL. The ideal candidate will provide high-quality patient care and rehabilitation services.
**Responsibilities:**
Conduct patient assessments and develop individualized treatment plans.
Implement therapeutic exercises and modalities to improve mobility and function.
Monitor patient progress and adjust treatment plans as necessary.
Educate patients and families on rehabilitation techniques and home exercises.
Maintain accurate patient records and documentation.
**Qualifications:**
Current Illinois Physical Therapy License.
Strong communication and interpersonal skills.
