We are seeking a dedicated Physical Therapist to join our home health team in Oklahoma City. The ideal candidate will provide high-quality, patient-centered care in a home setting.
**Responsibilities:**
Conduct comprehensive assessments of patients’ physical abilities.
Develop and implement individualized treatment plans.
Educate patients and families on rehabilitation exercises and techniques.
Monitor progress and adjust treatment plans as necessary.
Collaborate with healthcare professionals to ensure optimal patient outcomes.
**Qualifications:**
Valid Oklahoma PT license.
Experience in home health preferred.
Strong communication and interpersonal skills.
