We are seeking a dedicated Physical Therapist to provide outpatient care in patients’ homes.
**Responsibilities:**
Conduct comprehensive assessments and develop personalized treatment plans.
Implement therapeutic interventions to improve mobility and function.
Educate patients and families on exercises and wellness strategies.
Document patient progress and adjust treatment plans as needed.
Collaborate with healthcare teams to ensure optimal patient outcomes.
**Qualifications:**
Licensed Physical Therapist in Florida.
Strong communication and interpersonal skills.
Experience in home health preferred.
