We are seeking a compassionate Registered Nurse Hospice Case Manager to join our team in Aurora, CO. The ideal candidate will provide holistic care to patients and families facing end-of-life challenges.
**Responsibilities:**
Conduct comprehensive assessments and develop individualized care plans.
Coordinate with interdisciplinary teams to ensure quality patient care.
Educate patients and families on hospice services and resources.
Monitor patient progress and adjust care plans as needed.
Provide emotional support and counseling to patients and families.
**Qualifications:**
Current RN license in Colorado.
Experience in hospice or palliative care preferred.
Strong communication and organizational skills.
