We are seeking a compassionate Registered Nurse to join our Outpatient Hospice team. The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring comfort and dignity during end-of-life care.
**Responsibilities:**
Conduct patient assessments and develop care plans.
Administer medications and treatments as prescribed.
Educate patients and families on care procedures and resources.
Collaborate with interdisciplinary teams to optimize patient care.
Document patient progress and maintain accurate records.
**Qualifications:**
Current RN license in Ohio.
Experience in hospice or palliative care preferred.
Strong communication and interpersonal skills.
