**Salary:** $85,000 – $95,000 annually
**Job Description:**
We are seeking a dedicated RN Case Manager for our Home Health team in Grass Valley, CA. The ideal candidate will be responsible for:
Assessing patient needs and developing individualized care plans.
Coordinating with healthcare providers and family members to ensure optimal patient outcomes.
Monitoring patient progress and adjusting care plans as necessary.
Providing education and support to patients and caregivers.
Documenting patient care and maintaining accurate records.
**Qualifications:**
– Current RN license in California
– Previous home health experience preferred
– Strong communication and organizational skills
