We are seeking a dedicated Home Health Physical Therapist Assistant to join our team in Sonoma, CA. The ideal candidate will assist licensed physical therapists in providing quality care to patients in their homes.
**Responsibilities:**
Implement treatment plans as directed by physical therapists.
Monitor patient progress and report changes.
Educate patients and families on exercises and home care.
Maintain accurate documentation of patient care.
Ensure compliance with safety and health regulations.
**Qualifications:**
Valid PTA license in California.
Strong communication and interpersonal skills.
Experience in home health preferred.
